Registration form
To register for the 2009/2010 season, please
download the following registration form, complete as necessary,
attach the required cheques, and return via mail to:

Please note the fee changes depending upon
receipt of registration. Adobe Acrobat Reader is required
to download this document. We have provided a link to the
download site if you do not have the reader.
Refund Policy: A 100% refund is available
up to the first game. 75% will be refunded after the first
game, 50% after second game and 25% after the third game.
There will be no refunds after the game three, unless there
are extenuating circumstances.
U11 must submit a copy of their Birth Certificate
with their registration form. U12 must submit a 1"x1"
picture with their registration form.
